3. Appointment Policy
Appointments may be scheduled online, by phone, or via text message. See our Cancellation Policy for our notice requirements and how late cancellations and no-shows are handled. Collective Wellness reserves the right to refuse service to anyone for any lawful reason.
4. Treatment Consent
All patients are required to complete a health intake form and sign an informed consent prior to receiving any treatment. By proceeding with any service, you confirm that you have disclosed all relevant health information and understand the nature, risks, and expected outcomes of your treatment.
5. Payment & Refund Policy
Payment is due at the time of service unless prior arrangements have been made. We accept major credit cards and other payment methods as posted in our office. All sales are final. We do not offer refunds on services rendered. Unused prepaid packages or credits may be transferable at our discretion — please contact us directly to discuss your situation.
6. SMS Communications — Terms of Use
By providing your mobile phone number to Collective Wellness and opting in to SMS communications, you agree to receive text messages from us including appointment reminders, promotional offers, and direct two-way communication with our team. We do not share your SMS opt-in information or phone number with third parties for marketing purposes.
